Home care license renewal, change of ownership, and expansion

In short: we handle the renewal filing, the change of ownership notices, and the applications you need to add counties, service categories, or a second state, so your license never lapses and your Medicaid contracts never pause.

A lapsed license is worse than a late one

Renewal feels routine until it is not. The administrator who handled it last year left. The state moved the process to a new online portal. A caregiver's annual training hours were never logged. The renewal packet goes in late, or incomplete, and now you are operating on an expired license with Medicaid billing at risk and a surveyor on the way.

Change of ownership and expansion carry the same trap. Most states treat a new owner, a new service category, or a new service area as a new application in all but name, with its own notice periods and its own inspection. Owners who treat it as a formality discover otherwise when the agency rejects the filing.

What we handle

Annual and biennial renewals

Deadline tracking, renewal packet preparation, fee submission, and a prerenewal file check so the agency finds nothing when it looks.

Change of ownership

Notice filings, new owner disclosures and clearances, updated governance documents, and the Medicaid provider record updates that go with them.

Expansion

Adding counties or service areas, adding a service category (for example, personal care to a companion only license), opening a branch, or entering a second state we serve.

Included in every renewal engagement

  • Renewal calendar with every state, Medicaid, and insurance date in one place
  • Prerenewal audit of caregiver files, training logs, and clearances
  • Updated policy manual reflecting any rule changes since your last filing
  • Renewal application prepared, reviewed, and submitted
  • Response to any agency questions or deficiency notices
  • Confirmation of the new license and updated records with your Medicaid managed care plans

How it works

  1. Renewal review call

Free. We look at your current license, expiration date, and what has changed since you were approved.

  2. Gap list and quote

What is missing, what has changed in your state's rules, and a fixed price to close the gaps and file.

  3. Fix and file

We update files and documents, prepare the renewal or change filing, and submit it ahead of the deadline.

  4. Confirm and calendar

New license in hand, records updated, and next year's dates already on your calendar.

Common renewal and expansion mistakes we fix

  • Filing the renewal without updating the policy manual for rule changes
  • Missing annual in service or continuing education hours in caregiver files
  • Expired or never renewed background clearances
  • Treating a change of ownership as a name change instead of a regulated transfer
  • Providing a service category you are not licensed for after a quiet expansion
  • Forgetting to update the Medicaid provider record after a renewal or ownership change

Frequently asked questions

How early should I start my home care license renewal?

Start the file review at least 90 days before expiration. That leaves time to fix missing training records or clearances before the packet goes in. Some states require submission 60 days or more before expiration; we confirm your state's rule.

Can I keep operating if my renewal is late?

That depends on your state and whether you filed on time. Operating on an expired license can trigger penalties and Medicaid payment problems. Do not assume; call us and we will check your state's rule.

I am buying an existing agency. Does the license transfer?

Usually not automatically. Most states require a change of ownership filing, new owner clearances, and sometimes an inspection before the license is reissued to you. Plan for it in the purchase timeline.

Can you help me expand into another state?

Yes. A second state is a new license application, and we scope it the same way as a first license.

Do not let a date on a calendar put your agency at risk

Send us your expiration date. We will tell you, for free, what should already be in motion.
