Home care compliance consultant: audits, plans of correction, and survey readiness

In short: we audit your caregiver and client files against your state's regulation, fix what a surveyor would cite, write plans of correction that get accepted, set up training and clearance tracking, and handle Medicaid waiver provider enrollment.
Neatly organized caregiver files

You will not know your files are wrong until someone else opens them

Compliance problems are quiet. A caregiver started before the FBI clearance came back. The annual competency check was done but never signed. The client service plan was updated verbally and not in the chart. The TB screening is from the wrong year. None of it stops the agency running day to day, and all of it shows up on survey day.

A deficiency is not just a form to answer. It can mean a plan of correction with a deadline, a follow up visit, conditions on your license, and in serious cases a hold on new admissions or Medicaid billing. The owners who handle it well are the ones who found the problems first.

What we do

Compliance file audit

We review a sample of caregiver and client files against your state's regulation and survey tool, then give you a written findings list, ranked by risk, with the fix for each.

Plan of correction

If you have received a statement of deficiencies, we write the plan of correction the way agencies expect to see it: what happened, what you fixed, how you will monitor it, and by when.

Training and clearance tracking

A simple system for initial training, annual in service hours, competency checks, background and abuse registry clearances, TB screening, and license verifications, with dates that alert you before they lapse.

Medicaid waiver provider enrollment

Enrollment with your state's Medicaid program and managed care plans, including the program specific requirements (for example, Pennsylvania's Community HealthChoices, Ohio's PASSPORT, Virginia's DMAS, Washington's DSHS ALTSA).

How it works

  1. Tell us where you are

A scheduled survey, a deficiency letter, a Medicaid enrollment you have been putting off, or simply the feeling that your files are not as clean as they should be.

  2. Audit

We review files, policies, and logs against your state's regulation and give you a ranked findings report.

  3. Fix

We correct documents, write the plan of correction if needed, and set up the tracking system.

  4. Verify

A second pass confirms the fixes hold, and your compliance calendar is live.

Frequently asked questions

What is a plan of correction?

A written response to a statement of deficiencies that explains how each finding was corrected, how you will prevent it from recurring, who is responsible, and the completion date. Agencies can reject a vague one, so specificity matters.

How often should a home care agency audit its files?

At minimum quarterly for a sample of files, and a full review before any renewal or anticipated survey. New agencies benefit from a monthly check in the first year while habits form.

Can you help us enroll as a Medicaid waiver provider?

Yes. Each state's program has its own enrollment steps and standards beyond the state license. We prepare the enrollment and the policies the program requires.

Is a mock survey worth it?

For a first survey, almost always. It is the difference between learning the survey tool from us in a conference room and learning it from the surveyor in your office.

Do you provide legal advice?

No. We are consultants, not attorneys. For enforcement actions, license revocation proceedings, or litigation, we will tell you to involve a healthcare attorney and will work alongside them.
